Kōrero mō te tūranga - About the role
An exciting career opportunity has become available for a highly motivated and experienced person who will take a lead role in the coordination of short term home based support services for home help, meals on wheels and personal care services. The Integrated Services Assessor will, as required coordinate, implement and review packages of supports to ensure that people’s needs continue to be met. Where the client needs longer term care the Integrated Services Assessor will work alongside the Clinical Assessors, Care Managers and Team Leader Community Support Services to ensure that continuity of service and care is maintained where possible. Working within the Community Services Group, you will liaise with clinicians in the hospital environment and services in the community. You will be responsible for building partnerships and maintaining strong relationships.
This position if a 1.0 FTE, Job sharing would be considered for the right applicants.

Moū - About you
To be successful for this role you will:
- Hold a minimum of a level 4 qualification in health, allied or other relevant discipline
- Have strong cultural awareness and commitment to implement strategies and services that are responsive to the needs of Maori and other high needs populations
- Have clinical experience
- Have excellent written and oral communication skills.
- Be confident in the application of policy and process.
- Have high levels of computer literacy and ideally a knowledge of the InterRAI assessment tool
- Be committed to on-going quality improvement and high levels of service user and partner satisfaction.
