You will work alongside other clinicians, both within the triage team and across the wider community service, to triage the needs of people being referred to our service. We take a person-centred approach to caring for people referred to us, identifying when it - s important to talk to the person referred to hear their concerns and what they prioritise as most important to their wellbeing. This allows us the ability to work collaboratively with people around their self-identified needs, identify opportunities to provide early intervention from Triage and match their requirements to the team within our service who can best meet their need and pass the referral onward in a timely manner.

Mō tō mātou rōpū - About our team

The ORA Community Services work with people aged 16 and above with acquired neurological, long term, or age-related conditions, including stroke, dementia, frailty and post trauma conditions.  The teams are multidisciplinary including occupational therapy, physiotherapy, speech-language therapy, social work, dietetics, liaison nursing and allied health assistants, with close links with Nurse Practitioners, the AWHI (ESD) team, Geriatricians and the Rehab Physician.  There are three community teams geographically located in Wellington, Kenepuru and Kapiti.

A sound understanding of different disciplines, interdisciplinary working and delegating and working alongside allied health assistants will be of benefit.

This role would suit a candidate with strong communication and organizational skills as well as an ability to build relationships quickly with clients and their whanau.  It will suit someone who likes to look at people - s needs holistically, really listen to their concerns, and is passionate about our ability to make a difference to people - s lives.
