Clinical Assessors Community Health Integration Centre (CHIC)
Permanent Positions (1FTE in total, individual FTE negotiable (part time and full time)

Are you looking for an exciting and stimulating work environment, where you support a team who is leading a new future focused model of care?
We are currently seeking a suitably health professionals (with a current practising certificate) join Community Health Integration Centre (CHIC) Team. Established in 2019, the CHIC team is busier than ever, and we need to grow our team.
The Community Health Integration Centre (CHIC) provide a holistic patient clinical assessment and triage service for the Taranaki population, currently across multiple services with more coming on stream.
Clinical Assessors ensure that patients are assessed, triaged and connected to receive timely health care in the most appropriate setting They are responsible for ensuring quality services. There is a strong emphasis on developing collaborative relationships with internal stakeholders and partners, with the local communities, health and social agencies to strengthen the support for patients and their family/whanau.
Expert clinical judgement and knowledge of the Taranaki health landscape is essential as is a proactive approach to problem solving and achieving equitable patient centered solutions.
We are looking for health practitioners who understands the value of inter-professional working across different health professions and are committed to providing the highest levels of clinical care.
