This newly established senior role has been designed to support the Health of Older Persons Service in South Canterbury and to provide advanced clinical expertise in patient assessment, treatment and care. This is provided within an interdisciplinary model that best meets the patient requirements and aligns with the vision and value of service.
Day to day this role will facilitate high quality, culturally appropriate and well-coordinated care, and connect with primary, secondary, NGO - s and Aged Care Services.
The successful person in this role will demonstrate effective clinical management along with caseload management and utilise evidence based, advanced holistic assessment and diagnostic skills to diagnose and plan care. Additionally, the person will hold a Nurse Practitioner Qualification, be a member of a relevant national professional body and have advanced learning and contemporary knowledge for the health and rehabilitation of older persons.

- Ability to work across a wide range of patient settings and continuum of care
- Proven expertise and clinical assessment skills
- Function as an autonomous practitioner and anticipate and use initiative without direction
- Ability to self-evaluate and reflect on practice
