Are you looking for an exciting and challenging opportunity that will allow you to develop and extend your skills, within an established Complex Case management team?

We are seeking applications from experienced Registered Nurses looking to make Long Term Conditions and Complex Care their career direction.

You will be an integral part of our community health services, which is embarking on an exciting phase of transformational change, in order to deliver quality health services closer to home.  This includes implementing interdisciplinary and integrated approaches to patient and whaanau care.  Our innovative programmes include early supported discharge, rapid response, hospital in the home and reablement.  We are looking to increase capability and capacity to deliver these key programmes, working alongside our existing multi-disciplinary team including district nurses, allied health, and needs assessors.

The successful applicant will have a passion for working towards achieving equitable health outcomes for our most complex and high needs patients and whaanau.  The ability to develop effective working relationships with key stakeholders in both the primary and secondary settings is essential, as is the ability to work autonomously in a variety of community settings.  Strong problem solving, clinical assessment, along with effective communication skills are a necessity.

As a District Health Board we serve one of the largest and most diverse Maaori populations in the country. Achieving Maaori health equity is a key priority for us. Our commitment to this is driven by our desire to acknowledge and respect the Treaty of Waitangi and our belief that if we are serious about achieving health equity for our total population, we must first strengthen our commitment and drive to accelerate Maaori health gain in our community.

 

Essential:

  • NZ registration with a current APC with no conditions
  • 3 - 5 years post registration experience in relevant clinical setting
  • Current full NZ Drivers Licence
  • Effective communication skills (verbal and written)

Desired:

  • Postgraduate Diploma or currently studying towards this.
