Social Worker / Kaimahi Toko I te Ora (Medical Ward)

Service:  Medical Ward

Contract:  Permanent, Part Time (0.5FTE / 20 hours per week)

Salary:  As per MECA

Kōrero mō te tūranga - About the role

The Hutt Valley District Health Board seeks a qualified and registered social work professional for our medical ward.

You will be a key member of the multi-disciplinary team, providing comprehensive psychosocial and risk assessments, and ensuring safe and supported patient discharges. This is an exciting, fast paced, skilled social work role.  You will be working with patients and their family/whanau who are experiencing varied and often complex emotional, social and physical health related needs. The role requires creative, innovative thinking and problem solving.

This position is part time and consists of 0.5 FTE (20 hours) per week across Monday to Friday.  You will work with the other social workers on the medical ward to triage needs, allocate work and provide cross cover as required.

The Social Work Service is a supportive team, committed to professional development and continuous learning and clinical improvements. As a social worker within the service you will also provide cover to other parts of the hospital when required.

Moū - About you

The preferred applicant will have previous health experience, will be able to work comfortably within multiple systemic layers for the benefit of our patients, and have experience supporting patients and families with complex decision making and discharge planning. 

You must have excellent communication and interpersonal skills. You must have excellent time-management and triaging skills, be self-motivated and work well under pressure. You must be a team player and be skilled in developing relationships with a variety of hospital based health professionals, in addition to community based services and primary health care providers.
