Kōrero mō te tūranga About the role
We have an exciting opportunity for a Registered Nurse within Advancing Wellness at Home Initiative (AWHI) which is CCDHB s Early Supported Discharge service. Community Nurses in AWHI practice in an interprofessional model to deliver reablement and promote self-management of health for the people we work with in their own home.
The main focus of AWHI is to support people s reablement or maintain functional levels, after a hospital admission. A pull model is utilised to ensure we identify and effectively support people s needs on discharge from hospital and facilitate that to happen as early as possible providing system benefits to wider hospital flow.
AWHI works with people who have been admitted under general medicine and health of the older person s specialties from both Wellington and Kenepuru Hospitals. The team is split over 2 bases Ewart at Wellington Hospital site for the southern base and Kenepuru Hospital for the northern base. The 2 parts of the team cover all of CCDHB s catchment area between them, Seatoun up to Peka Peka but each team member has an identified base. This role is in our northern team based at Kenepuru.

You will be joining a friendly, fun and enthusiastic team from a mix of professional groups. We offer on-going professional development opportunities including inter professional support and training with the team of approximately 24 FTE around you, professional support and access to profession specific training though the Associate Director of Nursing lead. Cultural professional supervision is available from Māori and Pacific Health teams.

You will have experience as a community nurse and/or working with people who were admitted to hospital under general medicine or health of the older person, you will enjoy supporting people to manage their own health at home.
