About the Role;
We have an exciting opportunity for a suitably qualified and experienced person to embrace a role where you can be part of making a difference. You will provide service co-ordination for adult and older adult tāngata whaiora in the acute/continuing care continuum (including IPU, sub-acute, respite, residential, aged care) promoting timely and effective access to, and transition between, services throughout Te Tai Tokerau.
Utilising your previous experience in the health sector you will monitor and provide an understanding of service capacity (including occupancy, utilisation, referrals, vacancies) in the acute / continuing care continuum, in order to inform and facilitate care planning and service delivery.
Our goal is to ensure tāngata whaiora referred to the service receive timely access to the best matched service option, based on personal service needs, individual choices, service availability and contracted level of care within Te Tai Tokerau.
The role is part of the Adult Community Mental Health and Addictions Services, based in Kamo. This is a permanent, full time (40 hours per week) role with no weekend work.
About You:
You will have:
- A tertiary qualification in a health related discipline preferably with a professional qualification in nursing, social work or occupational therapy
- Experience working in the mental health and / or addictions sector
- Proven ability to develop options for people that reflect the principles of recovery
- Proven ability to work autonomously
- Proven ability to develop effective networks
- Ability to meet deadlines and to prioritise effectively
- Hold a current full drivers licence with the ability to travel throughout the region
