Rotorua Adult Community Mental Health & Addicition Services, Continuing Care Team

Mō te tūranga / About the role

• Part time (0.8 FTE available in total)
• Up to 32 hours per week, working business hours (Mon-Fri 8am to 4.30pm)
• Provide specialist intervention for people to optimise recovery and quality of life as well as case coordination
• Competent in all facets of assesment, holistic care and multi-disciplinary team work and have demonstrated ability to work with all ethnicities, particularly Māori/Iwi.
• To provide community mental health care based on current best practice and collaborative working within the inter-disciplinary team.
• Enusre all inter-disciplinary referrals within the team will be performed with consent from whaiora and their whānau (where applicable)
• Work collaboratively with complementary internal and external partner organisations.

Mō koe / About you

• Registered Clinician with experience in a community mental health environment
• Sound assesment and psychosocial intervention skills
• Annual Practising Certificate & Registered with the appropriate regulatory body

Awhe utu / Salary range

As per relevant SECA (Single Employment Collective Agreement), based on qualifications and prior experience
