Te Kaahui Ora - Maaori Health is looking for a Social Worker who is passionate about accelerating Maaori Health Gain and supporting better outcomes for patients and their whaanau while they are in hospital. Knowledge and experience working with psychosocial therapeutic frameworks from a kaupapa Maaori perspective and using a whaanau centered approach to strengthen patients and their whaanau’s capability and capacity to self-determine is essential.

You will be responsible for engaging, assessing, supporting, facilitating and working with those who are referred to Te Kaahui Ora - Maaori Health service.  Your exceptional interpersonal skills will see you working closely with a dynamic team of nurses, advanced care practitioners, kaimanaaki and the wider community to achieve whaanau directed goals, using a case coordination approach. An excellent understanding of, and commitment to, the Articles of Te Tiriti O Waitangi, and an understanding of te reo me ona tikanga is also essential.

You will support the seamless transition for Maaori from hospital to the community.

 

Essential

  • Recognised social work qualification and registration through SWRB
  • Current Practising Certificate
  • Experience working in a health setting
  • Full NZ driver's licence
  • Knowledge of Kaupapa Maaori models of practice
  • A minimum of 2 years post graduate experience
  • Fluency in Te Reo Maaori is definitely an advantage
