About the role
The Health Coach role will be working with the entire general practice team, including GP's, Nurses, Mind Health Practitioners, Community Support Navigators, Psychiatrists, Psychologists, Cultural Navigators and Whanau Ora. The right person for the role will support patients/clients and their family through the development of skills, tools, confidence, and lifestyle choices that enable the achievement of wellness goals. Conversations around mental health, providing emotional support and linking families into other services relevant to wellness is also a key element of the role. The Fono is the leading service provider for Pacific families and the Health Coach role is aligned with Pacific models of care. The role sits within a skilled and trained workforce of practitioners who understand the Pacific context, values, uniqueness, and diversity.
About you
This is a rewarding opportunity for someone with a suitable health, mental health or social services background and a passion for working alongside Pacific people and their families. You will be comfortable working in a role requiring flexibility, and variety of approaches that are appropriate for our patients/consumers. You will see this as a tremendous opportunity to use your skills to make a difference in the physical and mental health of clients and patients you meet.
You will have:
- Health Coach training experience.
- A broad understanding of health, social services, and mental health challenges within Pacific communities.
- Knowledge of protocols amongst Pacific peoples with the ability to work across various Pacific groups.
- A belief and faith that with support our patients and their family can make the changes required to increase their health, wellness and longevity.
- A positive ‘can-do' attitude, particularly in times of stress.
